Relaxation
Being fifty can surely tire a person out, so why not give them a free spa weekend complete with all the massages and facials that go along with it. You can also give them a gift certificate from their favorite restaurant or may be giving them a weekend at their favorite bed and breakfast. How about a weekend at a five star hotel? Why not give them an all expense paid cruise trip or may be a vacation into one of those tropical islands, or may be a country they havent visited before. 
Fun and Excitement
Being old does not mean you have to be boring. You can give them experiences that they have never tried before but always thought of. A word of caution, consider their health before giving them any of these gifts. A romantic hot air balloon ride may be just the thing the birthday celebrant needs to uplift his or her spirits. Why not give them a sky diving lesson or may be underwater diving lessons? A luxurious and comfortable limousine ride with all the kinks may also be fun for some. Im sure your grandpa will definitely feel the excitement in his hot new sports car. 
What Fits Their Character?
You can also go very personal and think of what they want or need. A coffee lover would absolutely adore a basket full of exotic coffee beans and may be a high tech espresso machine. You can even throw in a trip to a coffee plantation. A wine aficionado would love really fine wine and a tour of his favorite winery. A sewer will adore new sewing equipment whereas a gardener will probably enjoy some rare plant or flower.
